tailored shirts – how hard, how expensive?

I have long-ass arms and narrow shoulders. If a dressy shirt fits me in the shoulders it is Invariably too short in the sleeves, especially after I wash the shirt and it shrinks. I end up needing to roll the sleeves up, or risk looking like I’m wearing a shirt I outgrew, lol.

I know that a tailor can let out the sleeves of a suit to match your arms, somewhat, but I don’t imagine shirt cuffs are quite so easily fixable….are they? How hard is it to get a shirt fitted? Should I go with a shirt that’s right in the sleeves but bigger in the chest and get the torso brought in, or….what?
